Minor Media Release and Consent Form

Photo & Video Consent for Minors

I, the parent or legal guardian, hereby consent to the photographing and/or video recording of my minor child by One Cute Cookie Kitchen during participation in classes, programs, or events.

 

I authorize One Cute Cookie Kitchen, its owners, representatives, and employees, to use, reproduce, edit, publish, and distribute such photographs and/or video recordings for lawful business purposes, including but not limited

to marketing and promotional materials, social media platforms, website content, digital or print advertising, and educational or instructional materials.

 

I understand and agree that:

- My child’s last name and other identifying personal information will not be used

- Images will be used in a respectful and appropriate manner

- Media will not be sold to third parties

- I will receive no compensation for the use of such media

- This consent is voluntary

- Consent may be revoked in writing for future use only

- This release is binding and irrevocable for media already publishedI hereby release and hold harmless One Cute Cookie Kitchen from any and all claims, demands, or causes of action arising out of the use of such media.
